By submitting this application online, I/we, franchisee(s) of the 7-Eleven store located above have joined the 7-Eleven Franchise Owner's Association of the South Florida and hereby authorize 7‐Eleven, Inc to charge monthly dues of $30/month (plus $10/month for each additional store if applicable) to my open account #79. This authorization is to be effective until written notification by me/us to terminate this deduction.
